  • Upon confirmation of payment, pre-registration will be completed.
  • No cancelations or refunds will be accepted after pre-registration; please be sure not to register twice in error.
  • After registering online, a notification e-mail regarding your pre-registration will be sent, so please print it out and bring it with you on the day of the conference. In exchange, an attendance card (name card) will be handed out at the reception for overseas registrations.
  • Without payment of the attendance fee, the pre-registration application form by itself is invalid.
  • Please note that the attendance fee will not be refunded after payment.

Lump-sum payment only, installment payments not available.

VISA Requirements

A valid passport is required for entry into Japan. The Japanese Embassy or Consulate in your home country will be able to provide specific details about any entry visa requirements and applications.
Depending on the country, visa applications can take up to three to four months to be processed. It is therefore recommended that participants contact their local Japanese Consular Offices to determine whether or not a visa is required.

Please fill in and send the 'VISA INFORMATION FORM' to the Administration Office for Congress of the 20th Japanese Association of Cardiovascular Intervention and Therapeutics by June 30, 2011 by Fax or E-mail.

Please note that the Administration Office will prepare visa application documents only when an applicant provides the Secretariat with advance notification. In addition, the applicant must complete the Meeting registration in order to receive the visa application documents.
